You can get support from our service by making a referral.

A referral is a form that gives us the information we need to support you. Young people aged 13+ will need to consent to any referral made on their behalf and a parent or carer will need to consent to the referral if the young person is under 13.

You can refer yourself or someone else to the service by:

Once you reach out, a friendly team member will contact you to discuss the next steps and arrange your first session. All you need to bring is yourself and any goals or questions you have.

Please note: We are no longer able to accept referrals for tobacco use or nicotine vaping. For support with this, please contact Fitter Futures Warwickshire or the young person’s GP. We will continue to accept referrals for young people who are vaping other substances, such as THC.
