Refer yourself or someone else for support

The referral form below asks the key questions that we need to know to set someone up with the right support.

Once you’ve completed the form, someone from the service will contact you to discuss the next steps.

You can also attend one of our walk-in sessions if you are an adult over 25 years old and would rather not provide your details on the form, or you are looking to get support more quickly. These sessions offer the chance to talk to an Intervention Worker and a Nurse about making the first steps.

We need this information to get you the right support.

The information you give us is confidential, but there are times we may need to share your information with other services.

We will only ever share information if there’s a serious risk to you or someone else.

If you want more information or to get anonymous advice before you proceed you can:

  • contact us using any of the contact methods on our main service page.
  • speak to us when we contact you about your referral

We will always take whatever steps we can to keep information safe.
