The form below asks the key questions that we need to know to set you up with the right support if you are aged 25 or over.
If you are under 25, please use the form for our Eclypse Manchester young people's service. Professionals can also use this form to refer a young person.
If you are a professional and would like to refer someone over 25, please use our professionals referral form.
Once you’ve completed the form, someone from the service will contact you to discuss the next steps.
You can also attend one of our walk-in sessions if you are over 25 years old and would rather not complete the form, or would like to get support more quickly. In these sessions, you can talk to an Intervention Worker and a Nurse about making the first steps.
Drop‑in assessment times:
- Zion: Tuesdays 12–3pm
- Old School House: Thursdays 10am–1pm
- Bradnor Point: Fridays 10am–1pm
- North City Hive: Monday 10am – 1pm
