Change Grow Live Southampton supports adults aged 25 and over, as well as younger people who are rough sleeping or on probation. If you would like to refer someone under the age of 25, please contact the young person's support service No Limits on 02380 224 224. They can offer support for issues related to substance use.

Change Grow Live Southampton covers postcodes between SO14 and SO19. For assistance with substance use outside this area, please explore your options via NHS inclusion or your local provider. If you would like help with this, call us on 02380 717171 and we can point you in the right direction.

We value our partnerships with professionals who refer individuals for support with drug and alcohol use. We recognise that many people seeking help may have experienced trauma, and we are committed to ensuring our referral process is respectful, compassionate, and empowering.

Making a Referral

Our online referral process is straightforward and sensitive to the needs of both the referring professional and the individual being referred. To proceed, please:

• Agree for us to process your professional information.

• Ensure the individual has given their permission for us to receive and process their information, and to contact them using the details provided. This is a important part of the engagement process and helps to get us off to a positive start as we engage them.

• Complete a short referral form on their behalf.

The form takes about 5 minutes to complete and helps us understand the individual’s needs, including any factors that may impact their wellbeing or engagement with services.
