The Connecting community Team (CCT) have been funded as part of the Drug Recovery Prison (DRP) initiative at HMP Holme House. We have been commissioned to support men experiencing substance misuse and or alcohol issues in custody, approaching their release, through the gate into their home areas. Our aim is to create opportunities for the men to achieve their full potential by supporting them with a strengths based recovery approach. We work closely with partner agencies to help create a healthier and safer community both within the prison and in the home areas of the North East as part of the DRP and the continuity of care.
